Supporting SBT’s Specialised Care Needs

In March 2023, Nutrix Homecare began supporting SBT, a young girl with a progressive neurometabolic condition, following her discharge from hospital. Since January 2022, SBT had experienced ongoing deterioration, requiring specialised care to support her well-being and development.

Our dedicated clinical team conducted a thorough assessment of SBT’s needs, establishing a tailored care plan that prioritised her physical, emotional, and cognitive support.

Returning to Education with Bespoke Nursing Support

By January 2024, Nutrix Homecare introduced a bespoke nursing team, which enabled SBT to resume school on a part-time basis. This marked a significant milestone in her journey, allowing her to reconnect with peers and participate in school activities.

Our team worked closely with SBT’s family and educators to ensure her school environment accommodated her unique needs, creating a positive and inclusive experience.

Full-Time School Attendance Achieved

In March 2024, following the success of her part-time school attendance, SBT transitioned to full-time education. This achievement allowed her to enjoy her favourite subjects, participate in preferred activities, and strengthen friendships.

Nutrix Homecare's collaborative approach ensured her care plan seamlessly supported this progression, fostering independence and confidence in her daily life.

Transition to Carer-Led Support

By September 2024, SBT’s care needs had stabilised significantly due to the success of her tailored support and reintegration into school life. As a result, her care requirements transitioned from nursing-led to carer-led support, reflecting her improved stability and independence.

Nutrix Homecare continues to prioritise SBT’s well-being, maintaining open communication with her family to ensure her ongoing needs are met.
