Could You Be Eligible for NHS-Funded Care with Nutrix Homecare?

You may be eligible for fully funded care, either through the NHS or via your local authority’s social services.

Funding eligibility depends on the nature, complexity, intensity, and unpredictability of your needs, rather than on any specific diagnosis. Depending on your circumstances, your care may fall under NHS Continuing Healthcare (CHC) or local council-funded social care.

If you qualify for NHS Continuing Healthcare, your care costs will be covered in full by the NHS. If your needs fall under social services, your local authority will assess what support can be provided and whether any financial contribution is required.

What is NHS Continuing Healthcare (CHC)

NHS Continuing Healthcare (CHC) is a package of care arranged and funded entirely by the NHS for individuals with significant ongoing health needs. It is managed locally by your Integrated Care Board (ICB).

How the Application Process Works

  1. Initial Screening — “Checklist”

The process begins once an initial screening checklist is completed (often by a GP, district nurse, social worker or hospital discharge team). This will be carried out to assess whether you should proceed to a full eligibility assessment.

A positive screening doesn’t guarantee funding, it simply means you’ll be considered for a full assessment.

  1. Full Assessment by a Multidisciplinary Team (MDT)

If the initial checklist flags possible eligibility, a multidisciplinary team, typically including health and social-care professionals, will carry out a detailed review. They use a recognised “Decision Support Tool” to assess your needs across domains such as mobility, nutrition, skin/wounds, breathing, continence, cognition, communication, behaviour, medication needs, emotional wellbeing, and more.

Each area is graded (for example: “priority”, “severe”, “high”, “moderate”, “low” or “no needs”). If you have at least one “priority” need, or severe needs in two areas, you will usually be considered eligible. In some cases, a combination of moderately serious needs across areas can also qualify.

  1. Decision & Notification

The responsible NHS organisation, or your local Integrated Care Board (ICB), should decide within approximately 28 days of receiving your completed assessment.

If successful, you will be informed in writing and given a full care plan. If not, you can still request a review or explore alternative support with your local council.

Obtaining Support Through Local Authority Social Services

Not everyone will qualify for NHS Continuing Healthcare. In these cases, care and support are often provided through local authority social services.

Social services can arrange support such as personal care, home support, and community-based services following a needs assessment.
